Absence Excuse Form 

Attendance 9th – 11 th grade:

* e-mail information about absences to sena_cg@aps.edu

* fax an excuse note to 761-8421 or 761-8429

* write an excuse note and send with a student when he/she returns to school. The note needs to be placed in the basket on Cecilia Sena’s desk.

Attendance 12 th grade:

*  fax an excuse note to 761-8429 or 761-8421

* write an excuse note and send with a student when he/she returns to school. The note needs to be placed in the basket on Genevieve Molina’s desk.

REMEMBER – Attendance is the number one school success factor!

When students miss school days, they miss important lecture, notes, quizzes, test, assignments, or discussions in the classroom. Stay in school. It is important!
